I think the worst safety violation i have ever seen was back when i was a teen. My uncle owned an auto repair shop and we would often drink in there at night. One night my cousin got completely sauced and we strung him up on a cherry-picker using a pair of jumper cables. yep, not much for youth to do in the back country of Appalachia... Especially the guy who dodges it initially. Side note on forklifts, I know a guy who got ran over with one. It isn't a good situation to be in FYI.
