Title: Movie from 2005-11. Main character stabs wife and child in final scene [spoilers

It came out I think between 2005-2011. The main character is a hit man or involved in a gang or with the government and is commissioned for a job. He uncovers this guy is part of an underground child exploitation ring then decides to take down the organization on his own. The movie gets darker as he toutures (with hammer in kitchen) and murders members of the group. His family starts to be threatened (dead animal hung outside of house) and wife and son head to another house to stay safe. Then a plot twist! The main character is actually being initiated into some weird sex/violence cult. They chase him naked except for masks through a tunnel system. They force him to knife fight a cloaked figure to death. He wins, Cloaked figure is his son hunchbacked on his wife. He gets into cult, certainly no going back.
The movie is in English but I don't think it's American. I would say a psychological thriller/ horror.

I remember so many details but no specifics. Please help!

You ain't kiddin' there, my friend!  It's a great flick, but with the name, I half expected a direct to video clunker with latter-day Dolph Lundgren...
