I'm afraid I can't answer many of your questions, but I do have a possible answer for your 2nd question. While there have been many claims as to who invented the original hamburger, one of the claims is for a man named Otto Kause who lived in Hamburg, Germany, at the time of his suppose invention of the hamburger.
