The needles in AUDITION.

Someone pointed out that images of pain are much more effective when they are on a very small scale we can identify with.  We can laugh when we see a head being thrown from it's body, but we wince if we see a closeup of a needle piercing skin.

That reminds me: the "Is it safe?" scene from MARATHON MAN.

In "The Matrix" when Neo is sliding down that 'chute' while being flushed out of the power plant. He has all those weird implants along his spine, and he's pretty much sliding on them.

Decker getting his fingers broken in Blade Runner is way up there.

Take one sunken bathtub, toss in one boy scout, who's not very bright, add one blowjob from a woman with two inch fangs, and you have a moment that really, really hurts. From "Lair of the White Worm."

I think that moment in Blood Beach where the guy gets his junk bitten off had to hurt.  Oh well, it was attempting to rape a girl so it was alright.

We live in quite an interesting age. You can tell someone's sexual orientation and level of education from just their interests.
