Dagoth Ur in ELDER SCROLLS III - MORROWIND.
You hear the whole time about how he is "The Devil and the Enemy."
When you finally meet him, he has the coolest, most soothing voice ever.
Then you realize he is completely and utterly mad.

The Blind Demon in Fatal Frame.  Theres one point where she runs down the stairs and it literally made me jump when I first played it.  That game is scary as f*ck man.

Every boss is great on MSG, you have to approach them all with different tactics. It's the closest a video game came to truly emulating a serious action movie IMO, the finale against Rex and Solid Snake was computerised perfection.

The End in Metal Gear Solid 3 was pretty cool.  Big - and somewhat realistic - battle against a sniper in this huge forested area.  It could take hours. 

Project Snowblind had a cool one as well.  You fought this invisible guy, but one weapon could fire an electrified bolo type thing, and it would wrap him up and you could then tell where he was.  I'd fire dozens of those things at him until he was all glowing and sizzling from top to bottom   TeddyR

Pretty much any boss from the Gargoyle's Quest trilogy (especially Demon's Crest), Castlevania series, or Breath of Fire series.  Well, some in BoF are kinda lame (the "gremlin" bosses in the first one, for instance), but others are pretty mean looking (final boss of BoF III).
