scary black man driving an ambulance?

ok guys here goes!
i kind of remember a few things from a film i watched back in the 80's when i was younger,im 30 now.
the things i remember are that there was a black man (i beleive looked quite scary) driving an ambulance at a car with a man in it(i beleive it was dark and the scene took place on a bridge maybe even in the rain)?
some distinctive music was playing at this point and the camera focused on the face of the black actor driving the ambulance.
now i dont know weather this next bit is from the same film but i think so so it might help.
a white man was injured and barely survived,he was taken in and trained to extreme physical peak by someone.

any ideas?
