The "Final Grades" episode of THE WIRE.  The whole fourth season of that show is hard to take actually, with Prez & Bunny beating their heads against the public school system and Bodie's story arc ending in the most soul-crushing way.

The "Why We Fight" chapter on BAND OF BROTHERS. 
Also from BoB, I think it's the "Breaking Point" chapter, when Wild Bill & Toye get their legs blown off.

The episode Scott Tenorman must die from South Park.  I get that South Park can be incredibly mean-spirited at times and that Cartman is a character with little to no redeeming qualities. But the ending falls just on the side of too much for me.

The final episode of Scrubs entitled My Finale: heartbreaking yet uplifting.

The McCloud episode Someone's Out To Get Jenny: very scary but with a great ending.

The final episode of Twin Peaks: "How's Annie? How's Annie? How's Annie?"  Buggedout Buggedout

The Farscape episode A Constellation of Doubt. Season Four was mostly a dud, and Sci Fi made the right call to mercy kill it before it descended into Negative Space Wedgie of the Week Mode. But A Constellation of Doubt really takes the cake. Basically, it takes the basic concept of the Babylon 5 episode The Illusion of Truth (the characters are interviewed and have said interviews featured in a propaganda piece used against them) and does everything wrong that The Illusion of Truth does right. The one in The Illusion of Truth employs clever editing and the effective use of out of context quotes and images, as well as catching Dr. Franklin in a very unfortunate lie. The one in A Constellation of Doubt is so laughably hamhanded it seems doubtful it could convince even the most mentally deficient individual.
