I think if you were someone who never even heard of Star Wars before and watched the films in the order they were released (IV, V, VI, etc...) the ending of Empire Strikes Back would qualify as having a pretty good twist ending.

I mean throughout the first film and up until the moment Vader said, "No, I am your father" in the second film we were all led to believe that Anakin was someone else that Vader betrayed and murdered.  Not one and the same person.  I can imagine people that came completely out of left field for people seeing that the first time when the movie was first released.

I think Field of Dreams too.  It went right up to the end before Ray and the audience realized that when the voice said "If you build it he will come" he was not referring to Shoeless Joe, but instead to Ray's father John.  That was kind of a powerful moment for me, the little girl got something that those of us whose grandparents died before we were born or died when we were very young (like my Grandpa) never did - a chance to meet and even talk to her deceased Grandfather.

Other than those already mentioned, CHINATOWN's big reveal scene between Jack Nicholas and Faye Dunaway is a stunner. The twist in VERTIGO is kind of easy to spot but it's so psychologically and emotionally powerful it deserves mention.

The first time I ever watched SAW the twist at the end took my breath away!
JEEPERS CREEPERS had a nice twist at the end, too.

Andrew Davis' The Package (1989)

The above still is exactly what happens at the end of this film: quite unexpected.  Buggedout

I know a lot...and I mean A LOT of people disagree with me on this one...but The Mist was a great twist (or twisted ) ending.  But whether you love or hate the movie because of the ending, it really got people to react to it.  So it definitely had impact- which is better in my mind then a movie with a boring ending or a twist that just makes no sense in context.
