The cause of death is still tentative, but . . .?! preliminary results is that there was a fault in the gear shift of vehicle. The gear shift on that vehicle has the fault of showing the vehicle in park, when it's not really in park. Thus, with the engine running and the parking brake not on, the vehicle has a tendency sometimes to lurch forwards or backwards, sometimes with fatal results, as in this case.

Yeah, IIRC there was a recall of several hundred thousand affected vehicles.  It's worth noting this is why it's a good idea to use your parking brake even on an automatic - if your transmission fails in some way, due to a design flaw or just bad wear, it could save your life (or prevent vehicle damage if nothing else).  I'm bad about this myself, not trying to bother anyone about it.
