The ow!!! part of this is where one of the scumbags throws a punch at Clint who blocks the punch with his helmet, leaving the big tough guy screaming in pain and shaking his wrist. That never fails to make me smile. :teddyr:

Perhaps beyond "Ow", but the moment where a character tries to stand up after having both achilles' tendons severed in Hostel springs to mind. One of the most effectively awful and painful moments I can think of in a film.

That one also makes me think of the similar scene at the end of Pet Semetary. Yowtch.
The final trap in Saw V. Most of the Saw stuff didn't bug me, but that last sequence is just freaking painful to watch...

The needles in AUDITION.
Someone pointed out that images of pain are much more effective when they are on a very small scale we can identify with. We can laugh when we see a head being thrown from it's body, but we wince if we see a closeup of a needle piercing skin.
