Title: 1980's film with mad guy with hand coming out of head?

I say it on video around 1989 and I reckon it was maximum only a few year old - it's a sort of action adventure thing but there is a mad evil guy with a hand coming out of his head/ontop of his head - any ideas?

Off the top of my head (pardon the pun), it sounds like Jim Varney in "Dr. Otto and the Riddle of the Gloom Beam"
