Don't think the head is actually severed (if I remember from the book) and small fish had nibbled on the head allowing the eyeball to drop out.
Alternatively, the shark is the reincarnation of Jack the Ripper and has significant surgery skills. I mean if it is smart enough to figure out how to play a cello underwater, a bit of improvised surgery should be no problem.
