i went to high school with a guy named Richard Head. The school paged him to come to the office only one time calling him Dick. Even the teachers were rolling on the floor laughing when that happen. After that they always called him Richard.

Also knew a proctologist named Ivan Seymour Buttz. He took it in good humor and said with a name like his, his choice of profession was a given.
