The 2 final deaths in Deep Red..
One where a guy is accidentally dragged by a truck and deposited face-first into a curb, then his head is run over by a car!
The second, where a woman gets her necklace stuck in a lift, and it gets operated, pulling the necklace through her neck and cutting off her head!

Raiders of the Lost Ark: The scene where the big Nazi gets shredded by the airplane propellers, the driver gets crushed in half by the wheels of the truck, and of course where all the baddies melt and exploded by that angel of death.

Perfect Blue: The perverted photographer gets stabbed in the eye by a pair of scissors and then is stabbed a few more times.

Anaconda 3: One of the scientists is impaled by the cheapest looking CGI tail ever with blood spray looking like melted chocolate.

X-Men: First Class: Magento pushes the silver coin through Sebastian Shaw's head slowly.
