15. Sticking with Peter Greenaway, this movie went "too far" and probably killed his career.

A woman is gang-raped to death (offscreen, but still...) Released unrated in the US, I am sure it had an "adults only" tag of some sort.

I did not find one.  I checked out several US posters, watched trailers, and I guess it didn't have anything like that nor an NC-17 rating.  No doubt it would have deserved one, as would have the movie ANTICHRIST (2009), if you've ever heard of that one.  It had plenty of creepy sexual and violent imagery.  As steeped in controversy as it was, it didn't have a rating or "No One Under 17" type of blurb either.  So I don't think either should really count, sorry.

May I suggest another (fairly less) controversial religion-related film with according criteria for this thread:
THE DEVILS (1971)
A priest with unorthodox teachings stirs erotic passion within nuns of a 16th century French church.  A power-mad Cardinal seeks to remove him by any means necessary.  Released in the US with an X rating.  Stars Oliver Reed and Vanessa Redgrave!

I imagine both BABY OF MACON and ANTICHRIST had "adults only" notices in the US, but I can't find the evidence. (I saw ANTICHRIST in a theater, and a guy wandered in a little late during an explicit sex scene and said in a loud voice, "Is this a porno?")

This is an 18+ poster from Australia but I take it that won't count:

Oh well, we can still get to 100.

Yeah, trying to go by US criteria.. no worries though, there is plenty of helpful lists of at least NC-17 films via Wikipedia.

Hmm.. I'll have to check out Baby Of Macon sometime.  Another one to check out is "In A Glass Cage".  Really messed up flick about an ex-Nazi and child molester in an iron lung.  A young caretaker offers his services and becomes even worse than the Nazi!  That is also not rated nor features any kind of "No One Under 17.." blurb, so it doesn't quite fit the criteria, unfortunately.  But if you like really disturbing films, check it out.

^ Yeah, that one is pretty nasty.
17. EVIL DEAD 2: DEAD BY DAWN
Almost more of a remake than a sequel.  Same cabin, evil spirits and final hero, but a bit different plot-wise.  Sam Raimi's flair for impressive effects and camera style is in full force, making this one of my ten favorite horror films ever.  Like the first film, heavy on the gore.  The blurb reads "This Film Contains Scenes Which May Be Too Intense For Persons Under The Age Of Seventeen", which is close enough for this topic's criteria IMHO.  Also presumably cut for an R rating.

18. The Dreamers (2003)

Rated NC-17 for sex. I haven't seen it, but it's supposed to be pretty good.

19. FRITZ THE CAT (1972)
The first cartoon to receive an X rating (though some earlier anime would later get one).  College cat experiences politics, violence, racial tensions, counterculture and lots of sex in the late 60s.  X for sex, nudity and violence.  Fritz was voiced by Skip Hinnant of the childrens' show THE ELECTRIC COMPANY!  Fritz creator Robert Crumb was unhappy with this iteration.

20.  Blue Is the Warmest Colour (2013)

Critical favorite but extremely boring. The lesbian sex scenes are fairly explicit, though.

21. BAD LIEUTENANT (1992)
Thoroughly corrupt cop (Harvey Keitel) does blow, cavorts with hookers, hits on underage girls and more.  During an investigation into the rape of a nun he finally has a crisis of conscience.  NC-17 for violence, sex, drugs, profanity and nudity.  Great performance by Keitel.  Directed by Abel Ferrara (Driller Killer, Ms. 45, King of New York).

22. A Dirty Shame (2004)

John Waters' final film as a director was a return to the outrageousness of his earliest days.

23. I DRINK YOUR BLOOD (1970)
Weirdo gorefest involving a group of murderous, drug-fueled Satanist hippies becoming infected by rabies, and spreading the madness to the locals.  Originally released as rated X for heavy gore, but theaters showing the film were given the okay by the producers to cut it however they wanted for an R rating.  Often shown back in the day as a double bill with the completely unrelated I EAT YOUR SKIN (filmed in 1964).

25. INSERTS (1976)
Director unable to transition from silent films to talkies in the 1930s turns to making pornographic films.  X for sex/nudity.  Stars Richard Dreyfuss, Bob Hoskins, Jessica Harper and Veronica Cartwright!  Generally a commercial and critical failure.
