Couple on deserted island

My memory of this movie is "sketchy"...

I'm trying to find the name of a movie about a couple (a man and a woman) who voluntarily decide to go and live on a deserted island (for a year?). At the stat, they are either lovers or good friends.  But after a while of harsh survival life, they wind up hating each other.  In one of the final scenes, we see them through the eyes of the people who rescue them - they (or it may be just the woman) are both on the verge of starvation, covered in sores and emaciated.

I'm not sure, but I think at one point the man leaves the island for a while to go to another, inhabited island where he takes on a job repairing boat engines (or something of that sort).

Rayo Casablanca is correct. It is "Castaway" with the delectable Amanda Donohoe and the not so delectable Oliver Reed. And based upon the book "Castaway" by Lucy Irvine, who is played by Donohoe in this film. Also see Donohoe in "Lair of the White Worm," which is reviewed at this site.
