For me, it's any scene involving the consumption of poop or puke.  The Pink Flamingoes scene made me gag but even worse was the puke slurping scene in Bad Taste.  That scene actually made me upchuck a little in my mouth.  I had to go to the bathrom and spit in the sink.

Lots of puke actually in believe it or not Problem Child 2...there's a gross scene involving a flying soiled diaper in an episode of "Mr. Bean" too.

Well, I don't know if it's the grossest scene of all time, but I have never seen anybody make it through the "custard scene" of Dead Alive unscathed. I've shown it to a lot of people, too.

It's not even the part where the ear falls in, it's the part where the unaware petitioner eats a streamful of bodily fluid.

I've seen all manner of people react to that scene, and it is universal disgust. I actually quite it enjoy it these days.
