80s horror movie with mirror (?) monster

Hey there. Useful Resource!
First post:
Been trying to find this for years as I never saw it as a kid. I found this at a videostore I used to go to between 1988-1990, but I believe the film was released sometime between 1980-1989. All I remember is the artwork for the VHS, being a kid (probably a boy) in front of shattered mirror (possibly) in which was a humanoid monster: sharp teeth, scaly Klingon-esque forehead with a third eye (possibly, again). I've tried a million searches over the years, but I can't seem to find anything. Help me see this movie, I think it's the one that got away!

Another hint: I believe a 'talisman' of some sort was involved as well.

Aw, geez.  I suddenly know exactly the movie box you're talking about.  But I can't think of the title!   

I'm pretty sure it was a Charles Band movie, so you may want to search for Full Moon Productions video boxes.
