Sound effects errors

Have you ever seen a movie where someone fires a gun or something, but you don't hear any sound? Or where the sound effect doesn't quite match what you see on the screen?

I remember a movie a few years ago (it might have been U.S. MARSHALS, but I'm not sure) where there is a gun battle.  You can clearly see a guy in the background fire a pistol (you can see the smoke shooting out of the barrel), but you don't hear a corresponding gunshot.

Can you think of any others like this?  It's much more interesting when these things happen in major releases as opposed to low-budget films, where you would expect to see mistakes like this.

I noticed two sound oddities in "Star Wars", which have probably been well documented.

One is the scene in the Death Star where Luke and Leia are about to swing across the chasm.  It seems as some normal gun sounds should been replaced by the laser effects, but weren't.

 Also, there's a scene between Vader and Tarkin where Vader's hand movements and the voice don't seem to be synched very well  You hear his voice and then when he stops talking, his hand moves as it should have when he was talking; looks akward him shaking his hand with no voice

I had the misfortune of watching Leaving Scars not too long ago (bad "thriller" movie) and the gunshots sounded like nothing more than one of those old cap pistols.  So ok, maybe you can't afford real blanks (or scriptwriters, or actors. . .) but doesn't Hollywood have a bank of sound effects somewhere?  How frippin' hard is it to just dub in some realistic gunshots?

One of the things I notice in some movies is that when charectors are talking there voice does not sync with there lips.For some movies its just a few scenes others its like the whole movie,and im not talking about Japanese movies,Im talking about english movies.I unfortuanatly can't think of any examples right now but I know I've seen many of these lip sync problems, im sure you guys have too.

In Evil Dead 2, when Linda's head is biting Ash's hand, he turns to the camera and says "Workshed!!!".  The amazing thing is that he does it without moving his lips at all, and he uses someone else's voice.
