Often requested, now finally available for purchase on Blu-ray. Here are the specs:

Are you using an actual poll of "most requested titles," or is this a personal opinion?

I'm more interested in TAMMY & THE T-REX, released at the same time by the same company.

Just an observation from hanging out at various horror message boards since 2001. Spookies was always one of the most requested, usually followed by Blue Monkey, Iced, Fatal Games and Eyes of Fire. This lineup hasn't changed much since 2001.

Last year an Indie label set up a go fund me page to get Blue Monkey released but the project was cancelled for unknown reasons. Apparently Iced exists only as a VHS master (which prevents a blu-ray release) and locating elements for Fatal Games seems impossible, since it never had an actual theatrical run. No idea about Eyes of Fire, Indie labels are kind of hush about it.
