It is tricky... I'm not so concerned about "Mr./Mrs./Dr.," but is Dr. No a real name, or just a nickname? (I don't remember.) The case I was on the fence about was superhero handles (i.e. Superman), which I think should not be allowed unless the hero uses a real name. I hadn't decided what to do in cases like this one, but I'll allow it.

(just my 2c but I think we should only resort to this kind of prefix stuff if we're struggling for straight ahead actual names tbh)
