The director, in a fit of jealous anger after finding out that the cameraman had been doing his wife (you boffed my wife?!), decided to get even with him by loading real rounds in Deniro's guns for this closeup shot. Unfortunately, at the preeminent moment, an unexpected and painful fart causes Bobby's aim to veer and he shoots and kills the soundman (stage right) and cue card guy (stage left).
