Well, death by chocking/strangling yourself on purpose while masturbating. It's very dangerous and most people either severely injure themselves or kill themselves as it seems to be what happened in Carradine's case.

That's messed up.
Apparently he took auto-erotic asphyxiation pretty seriously.

BANGKOK – Police are speculating that accidental suffocation, not suicide, may have caused the death of American cult actor David Carradine, whose body was found in a hotel closet in the Thai capital with a rope tied to his neck, wrist and genitals.
