I still can't fully comprend the scene where Pyramid Head rips the skin off of the girl.  That was crazy and disgusting, but pretty gory enough.

There's no movie called Rambo 4.  ;-p  I was referring to the fourth film, which is titled Rambo, at least in the USA.  Rambo IV is what is SHOULD have been called, or maybe John Rambo as was the initial choice.  The first movie, of course, is First Blood.  That film and the second and third films have very little gore, really.  Extremely confusing series of movie titles, I must say.  But yeah, Rambo is the goriest action film I've seen.
