That's the Penis Monster from Troma's Tromeo and Juliet. 

... unless Troma decided to put that thing in another movie as well.

I hate to go all necromancer on this thread, but could it perhaps be "Toxic Avenger 4: Citizen Toxie"? I saw the mention of Tromeo on there and that sprung this one to mind. I know that creature belongs to the villian, the Noxious Offender from "TA4".
