Sir, you are completely correct.  The town pictured on that map is Geraldtown.  It's where you start, and the first town destroyed by Mantor.

I believe this is the C-64 version, I know it was also available in a stand-up arcade cabinet, but I don't know of any others.

Yep, that's Satan's Hollow from Bally Midway.  Early 80's game. I pumped my share of quarters into it at the local arcade back in the day. Has some great artwork on the side too, of the devil holding a crystal ball.  I used to play Midway's other quarter muncher, Wizard Of Wor, as well.
