It never went away completely, however, it's generally controllable with modern medicines.

In the US you have a few deaths from this every year (normally less than twenty, although it is rare that a year passes without someone getting infected). The last large scale outbreak in the states (that I've heard of) was following the 1906 San Francisco earthquake, so yeah it has never quite gone away fully. Some strains of it are more treatable than others (although with antibiotics being outpaced by disease evolution and the rise of 'super bugs', this situation could change), but in general you are looking at an 11% mortality rate in a modern country.

If we had a major outbreak would we have enough medicines?  That's an issue I wonder about.

I've seen plenty of these signs when I was growing up.

Yup, I just grew up knowing the plague was still a thing and not to touch wild animals.

there were times when it hurt my heart that i couldn't help a baby whose mama had been killed, but the rule was to call animal control or fish& wildlife to handle it.

The scary thing about the black death is all it takes is for a couple dna base pairs to mutate and it could become a superbug again.
