In the original unreal, in a level called bluff eversmoking, you can see a napali cross on a distant mountain  peak. The cross looks like an x mounted up on a pleas the ma pali have 4 arms.

Use your sniper rifle on max zoom to look at it and you'll see a writhing na pali nailed to it.  Put him out of his misery with a mercy round,  one shot  will do,  hecs in bad shape.

Seconds later a massive lightning bolt will strike the ground and leave a massive health pack behind,  apparelty as a reward from the goddess vandora for ending the agony of a native.

I found this long ago and finally looked it up on a wiki. Originally I shot the native because he was clearky in agony and I could not read him to help. I was surprised to see the massive lighting strike and amazed to see a health pack left. I assumed it was for being merciful.

In ELDER SCROLLS IV: OBLIVION I found the floating body of a troll who had committed suicide.  He had left a note bewailing the fact that no one would pay the bridge toll because he was not scary enough.

In an expansion of Elder Scrolls III: Morrowind, I came across a journal that told of the construction of an experimental airship that crashed a generation before, taking the lives of those aboard, and in the ultimate Easter egg hunt, I matched some landmarks in the journal with the terrain on the southeast coast and found the bodies of the crash victims.

I remember that!  I also remember finding the skeleton of someone named "Indy" buried by a landslide in a tomb in that game, and the drop you got from him was "Indy's Lucky Hat."

In plansescape:torment you were  given the option of introducing yourself to numerous characters as "Adahn". If you do this enough times and go to the smoldering corpse bar you will meet a character called "Adahn the imagined" who is a very sad and tragic character that can't  remember his past or where he comes from and only knows his name is adahn.

He is bewildered,  confused,  helpless and you can scam him out of a ring, some gold, a dagger and some XP before he wanders  off helplessly  dazed and confused muttering to himself he can't remember any thing but his name. 

I never played it,  I read about it in an article on strange moments in games.

World of Warcraft had tons of stuff like these, but I guess any player from the game heard them a million times already.

The weirdest one which I actually found by myself back then was the "Cannibal Nanny" in Nagrand, Outlands. You can get there only by flying, only to find a lonely female troll living in the middle of a floating island, which ask you to keep quiet or you'll wake up the children. If you search her house, you do find toys and things like that lying around, but no children. There's also a dog house with no dog, but plenty of bones and blood in the entrance.
